Chapter 1 (probably the most important chapter) Community health nursing theories 1) Nightingale’s Theory of the Environment  focuses on the impact of a person’s environment on their health, focuses on preventative care/ highlights the 2)Health Belief Model  assumes a person’s primary motivation in taking positive health actions is to avoid getting a disease/ the likelihood that they are going to take these positive health actions is based on 4 different things -Modifying variables: age, gender, race, economy, education/ so depending on where they fall with those things, they may or may not be inclined to take positive health actions -Perceived severity & susceptibility of getting a disease: so if they perceive that a disease is very serious and they can easily get it, they’re going to be more motivated to take action -Perceived benefits vs. barriers of taking action: so if they see a lot of benefits and not a lot of barriers, they are more likely to take action and then the opposite would be true too -Cues to action: are they getting advice from their dr, seeing media campaigns (stop smoking), disease effect on family/friends, if they are getting inundated with all of these cues to action then that will increase the likelihood that they will take a positive action to improve their health Community Health Nursing- So within community health nursing, the client is the community or population as opposed to an individual -2 parts that fall under community health nursing: community based nursing and community oriented nursing Community based nursing – we are providing illness care/ so we are taking care of patients with acute or chronic condition and we are treating individuals and families/ Ex: home health nurse providing wound care, school nurse providing an epi pen injection for a pt. So they are doing hands on specific nursing care for individuals and families Community oriented nursing - this is not focused on illness care, they are focused on disease prevention, education. They are trying to improve the collective health of the community. So their focus will be on health education, health promotion and disease prevention. Ex: Public health nurses are community oriented nurses- they are not providing illness care , they are focused on disease prevention, health promotion, and education. Know the differences between community based and community oriented! Also know the difference between public health nursing and community health nursing. Community health nursing contains BOTH community based and oriented. So we are doing some illness care within community health nursing. However, with public health nursing, that is JUST community oriented. They are NOT doing illness care, or treating individuals or families. They are focused on the population and the community as a whole 4 ethical principles in community health nursing: Respect for autonomy- respecting a patient’s right to self-determination/ Ex: if they have cancer and they want to forgo treatment, that is their right Non-maleficence - not doing any harm to the pt Beneficence- doing what is best for the patient / maximize the benefits that you are providing to the patient Distributive justice – focused on fair allocation of resources in the community/ Ex: if you have a part of the community that is underserved and economically challenged, making sure that they are getting fair allocation of health care resources Epidemiology- the study of the spread, transmission and incidence of disease and injury Really important that you know the 3 components of the epidemiology triangle!
