How did the simulated experience of Red Yoder’s case make you feel?
I felt pretty good about the simulation. When we start a new unit, it always takes a minute to get used
to the options available, and where the controls are. A few of the desired actions were difficult to find,
and there were a few I didn’t find. I am going to run through the simulation again,so that the next Vsim
is a little smoother.
What nursing actions did you feel were appropriate within this scenario?
Education was key in this simulation. There were a lot of teaching points for this patient and his family.
Educating the patient on medication routine as well as wound management and diabetes education,
were all required. That is a lot of information for a patient in one sitting. So managing the delivery of
that information is important, so that the patient continues to listen. Especially a patient preparing for
discharge, they may not be listening as closely due to preoccupation of wanting to leave.

PCC/ What information should be included in the hand-off communication to the home health nurse
T&C about Red Yoder and his family for a successful transition from acute care to the home
environment?
Making the new caregiver information about the current wound, information about staging, how
deep, the care regimen ordered as well as education given to the patient and the status of
their ability to care for this wound. Diabetes management will be a topic that needs discussed
with continuing care. Giving the home health care professional about the trend of
hospitalizations regarding diabetes management, education and skill of the patient for insulin
administration. Sharing the baseline vital signs is important as well as sharing what family’s
participation will be like for this patient, who is getting involved, and if there was a schedule
discussed.
