Nursing practice issue related to wound care is proper management of wound developed in the hospital

among elderly patients (pressure ulcers).

Briefly describe your rationale for your topic selection. Include the scope of the issue/problem.

Pressure ulcers are one of the most common issues among elderly patients due to their inability to proper

change position while in bed either due to weakness, injury or deteriorating health condition. Proper care

put in place will help in the reduction of pressure ulcers among elderly patients because they often never

heal or continue to worsen as a result of poor healing wound process among the elderly. Delegating a

wound care team to oversee both prevention and treatment of pressure ulcers among hospitalized patients

are essential to help reduce occurrence and better improve patient’s outcome.

Star Point 2: Summary (Evidence to support need for a change)

Describe the practice problem in your own words and formulate your PICOT question.

Pressure ulcers developed in the hospital among elderly patient is a somewhat considered neglect

because it shows that patient has been in a particular position for an extended period of time. It is a form

of injury to the skin and has various stages depending on its depth; it is often associated with prolonged

hospital stay and causes pain, possible infection and can lead to death as well. Considering that this

increases the problem of a patient compared to the actual reason why they were hospitalized, it is

essential to have a specialized team to care for this ulcers, monitor the progress and prevent reoccurrence,

find out causes and plan ways to avoid it in other hospitalized elderly patients. One possible way to

achieve this will be developing a PICOT question and carrying out a study or research.

In hospitalized patients aged 50 and above who develop pressure ulcers (P) does frequent

repositioning (I) compared to not repositioning (C) help in reducing the development of pressure ulcers

(O) over a 90 day period (T)?

This article even though there wasn’t a final study making it a weak evidence strives to address the

importance of having a wound care team to help in the prevention and treatment of pressure ulcers. This

will go a long way in curtailing responsibilities of staff caring for patient, reduce incidence of neglect and

give a specialized team enough time to not only assess but also track the progress.
