Student Response: The use of my critical thinking skills in order to come to a conclusion due to the
process of elimination. Re-evaluation of some of Miss. Park's lifestyle and habits. Complete abdominal
exam.
Consisting of chief complaint, present medical history other than what we already knew due to chart
history. History of present illness, reviewing the system of the abdominal pain. It took me longer than
expected due to very di cult fficult with Shadow health logging me out at least twice this time. Obtaining
objective and subjective data. Along with inspection, and evaluation of abdominal assessment such as
inspection, palpation, percussion gave us a better insight on miss Park.
Explain the clinical reasoning behind your decisions and tasks.
Student Response: in order to rule out anything other than fecal impaction, the steps had to be
followed for an abdominal exam. Trying to maximize my time with the patient in order to come to a
conclusion of a diagnosis. based on the assessment gaining the ability to complete a proper abdominal
exam and document any normal and abnormal findings. used all data collection to establish a diagnosis,
assessment, and plan for the patient.

Student Response: As the weeks go by and we continue with shadow health I become more
comfortable with asking patient questions or when to ask questions, when to empathize and educate. It is
becoming more apparent that correctly documenting all data obtained and accurately will provide the best
outcome not only for our practice but for the patients.
