Sinusitis or rhinosinusitis is defined as inflammation of at least one but can involve all
the paranasal sinuses. Sinusitis can be either acute or chronic. The inflammation can be caused
by viral or bacterial pathogens. This paper will serve to identify how to determine whether the
etiology of infection is viral or bacterial, as well as to distinguish when to initiate antibiotic
therapy versus symptomatic treatment.
Sinusitis is a common complaint in primary care practices and results in more
antibiotic prescriptions than any other outpatient diagnosis (Ebell, McKay, Dale, Guilbault, &
Erimas, 2019). Acute respiratory infections are primarily viral in etiology; however, antibiotics
are often written (Shaver, Jacobs, LaMonte, & Noyes, 2019). Considering the major health
concern of antibiotic resistance, it is imperative that providers of care be good stewards in the
prescribing of antibiotic therapy.
The American Academy of Otolaryngology-Head and Neck Surgery (AAO-HNS)
clinical practice guidelines for adult sinusitis states that diagnosing acute sinusitis or
rhinosinusitis is based on the presenting signs and symptoms. These symptoms include the
presence of facial pain, pressure of one or more of the paranasal sinuses, and purulent drainage
(Rosenfeld et al., 2015). According to Aring and Chan (2016), an excellent indicator for the
presence of an acute bacterial sinusitis infection is the length of illness and the worsening of
symptoms. Viral sinusitis infections are ordinarily self-limiting, respond well to symptomatic
