Community Health and Population-Focused Nursing Field Experience - C229
B. Write your community health nursing diagnosis statement
Increased risk of exposure to Covid-19 among the elder population, related to knowledge
deficit of preventative measures, as evidence by increased numbers of Covid-19.
1. Explain how the health concern is linked to a health inequity or disparity within the
target population
The elder population is defined as 65 and older. Covid-19 seems to impact this group
more significantly than any other. This is typically due to decreased knowledge of preventative
measures but is also due to elderly having co-morbidities. The CDC states that you have a 5x
higher risk of hospitalization with covid-19 if 65-74 years old, 8x higher if 75-84 years old, and
13x higher if 85+ years old. As well, if you have other illnesses such as, diabetes, hypertension,
obesity, or cancer then your chance of having problems from this virus is significantly greater.
(Older Adults and COVID-19 2020). Tennessee is the number 1 state for new daily cases,
averaging 142.2 per 100,000 cases a day. My county within TN has a population of 28,020
people and 20.65% of those are 65 years or older. This means that almost a quarter of my
county’s population is already at an increased risk for not only exposure, but complications from
Covid-19. (CDC COVID Data Tracker 2020).

a. Discuss the primary community resources and primary prevention recourses
currently in place to address the health concern
I live in a very rural, small community, with little resources. The county Health
Department would be the main source of resources and they provide Covid-19 testing, teaching
about precautions, they provide masks for those in need and will eventually carry the vaccine.
The local doctors’ offices are also a source of resources because they too provide education,
screening, and masks.
b. Discuss the underlying causes of the health concern
The underlying cause of the health concern within my community would have to be the
lack of education. There are no billboards or ads in the newspaper about Covid-19 precautions.
Unless you are looking for education, you will not find it and that is an issue. The elderly are at
an increased risk but are still out and grocery shopping, attending sporting events, and not
maintaining social distancing at an elevated rate.
2. Discuss the evidence-based practice associated with the Field Experience topic
An article that covers evidence-based guidelines for preventing the spread of Covid-19
states that strict handwashing, social distancing, wearing a mask and self-quarantining is
essential. This article also states that while normal surgical masks have not been proven to
prevent the spread of the corona virus, they help. However, any medical professional within 6
feet of a positive case should wear a N95 face mask. In mid-January of 2020, China initiated
strict mandates for contentment strategies, including movement of people. This significantly
dropped their corona virus cases by almost 37% as compared to cities and countries without.
(Nicola et al., 2020). This proves that with strict social distancing, numbers will decrease.
