Let's talk about testosterone. What are some of the indications to give a patient testosterone
(i.e. what is testosterone supplementation used for) and what formulations is it available in?
- Testosterone supplementation may be used to treat primary hypogonadism or late
onset hypogonadism. These conditions manifest as delayed puberty in young boys,
infertility, sexual dysfunction or erectile dysfunction in adult men, and declining general
physical health and possibly cognition in older men. Testosterone supplementation is
available in oral, buccal, IM, transdermal patch and gel formulations.
Let's say that you have a child who presents with acute otitis media. Aside from any other
prescription that you may give the child, the mom wants to know what dose of ibuprofen
they can give the kid. What is the ibuprofen dose for a child who weighs 8 kg?
- The dose is 5-10 mg/kg. You should recommend the lowest possible dose but they can
safely use as much as the 10 mg/kg dose.
A few points to emphasize here are that ibuprofen is effective for pain control plus antiinflammatory effects and therefore provides better pain control for acute otitis media
than acetaminophen (tylenol). The correct dose, again as you said, is 5-10 mg/kg/dose
q6-8h. A child weighing 8 kg can receive 40-80 mg every 6-8 hrs as needed for pain.
5 mg/kg/dose x 8 kg patient = 40 mg
10 mg/kg/dose x 8 kg patient = 80 mg
so you can give 40-80 mg q6-8 hrs
