Pre-Assignment:

1. Trimethoprim/sulfamethoxazole (Bactrim, Septra, other)

Classification: Anti-infective

Chem. Class: Sulfonamide—miscellaneous

Use: UTI, otitis media, acute and chronic prostatitis, traveler’s diarrhea, chronic

bronchitis, pneumocystis jiroveci pneumonitis

Action: SMZ interferes with bacterial biosynthesis of proteins by competitive

antagonism of PABA when adequate levels are maintained; TMP blocks synthesis of

tertrahydrofolic acid

Route: PO medication

Metabolism: Liver

Excretion: Kidney

Dosage: adult: 160mg q12h X 10-14 days; child: 8mg/kg daily divided into 2

doses q12h

Time to Peak: 1-4 hours

Adverse Effects: headache, insomnia, allergic myocarditis, hepatitis, renal failure,

rash, dermatitis, cough, shortness of breath, anaphylaxis, Stevens-Johnson Syndrome, hemolytic

anemia, increased BUN

Contraindications: at term pregnancy, breastfeeding, infants <2months,

hypersensitivity, megaloblastic anemia, CCr< 15ml/min

Nursing Considerations: monitor I&O ratio, renal function tests, assess type of

infection, assess blood dyscrasias, rash, fever, sore throat, assess allergic reactions, teach use of

alternative contraceptives, take medication with full glass of water to prevent crystalluria, avoid

sunlight or use sunscreen, avoid OTC medications unless directed

2. Fosfomycin (Monurol)

Classification: anti-infective

Use: Uncomplicated UTI
